SURVEYS 2009

By participating in Surveys you help us gather vital information in relation to Water Safety and Drowning Prevention and Awareness.

Hannahs Foundation is currently seeking participants who have:

1. Recently Sold/Purchased/Rented a house and specifically with a pool in the last five years (please fill out Part ONE)

2. Experience a Drowning or Non Fatal drowning in a child under five (Please fill out Part TWO)

3. Children under five (please fill out part THREE)

4. Or all of the above (please fill out FULL)

 

Section One: this part of our survey will give us vital statistics on how many purchases/rentals of pools are provided with pool compliance certificates, how many are found to have faults after purchase.

Section Two: This sections comes with a strong warning that questions are emotional and relate to the death of child or non fatal immersion. Please if you are in doubt about answering ANY questions relating to this DO NOT FILL this form out.

Section Three: Relates to Children under five in your family, their swimming ability and regular swimming lessons, near immersions in swim classes and also if you have ceased swimming lessons due to costs and YOUR opinion on Water Familiarisation classes.

the documents have been saved with Track changes so any answer you give should highlight. If not please highlight your answers in RED and BOLD.

If sending by post please print forms off and circle the correct answers applicable to your situation. Please use correct postage on the envelope.

If emailing the surveys please DO NOT change the document name, we will register the files with a Survey number upon receipt.
